齒顎矯正治療 for MEN

    大約在三年前診所出現了為充滿自信、擁有皎好身形的男仕，因為覺得牙齒太過突出，來尋求矯正治療。和他相談甚歡的情形下，我也欣然的接下這個任務。

    

    經過我們徹底的分析與討論，事實上是非常辛苦的矯正治療，因為除了將牙齒排列整齊外，要移動到顱顏面理想的位置，包含他喜歡的角度、與笑容，這樣獨特性與客製化的過程，必須將上下顎前牙後移，讓上排牙齒露出夠多的牙齒方顯得陽光。

    將上下齒列移得夠遠，才能讓鼻唇型自然立體，而且在各個角度，尤其是與他人談話時的斜側面與正面，唇齒之間的完美韻動，才能讓我們的矯正治療達到完美境界。

  

   本文的第二個案例，患者有非常不整齊的排列，所以平常不太微笑，容易讓人覺得距離感，而真正瞭解他時，卻發現不整齊的牙齒和外突的臉型真的掩飾了他很有質感的內在。

    在10個月的矯正治療中，除了解決不整齊的上下齒列外，必須將上下牙齒中線排對齊，外突的犬齒向內靠近。原本下顎骨角度於肥厚的感覺，經由咬合平面的改正，讓它適度向前及向上移動，矯正時期咀嚼肌肉的改變，也讓臉型有變尖的感覺。

    沒有過度的將上下唇向內移動，以避免過於凹陷。較為對稱的唇型 (Lip Symmetry) 可以讓人留下較為帥氣且親切的印象，所以矯正事實上是必須符合個性，與達到患者本身的期望的。矯正治療中，除了必須傳遞應有的美感外，拉近人與人之間的距離感也是必須考量的。

    The following patient has very crowded upper and lower teeth. He told me that he doesn't like smiling and this easily makes him look indifferent. However, after treating him for a while, I noticed this bad alignment and protrusive face indeed disguised his personality and how good he is.

    This 10 months of treatment resolved his malocclusion by correcting his dental midline, moving the front teeth more backward. By means of changing the occlusal plane, the mandibular plane angle was reduced by making the it more forward and upward, called counter-clockwise rotation. I did not push the front teeth more inside, since the more friendly facial appearance can be created. Therefore, orthodontist actually can make the treatment plan fit the patients' personalities and fulfill what they really want.

   在決定拔牙之前，我們從最重要的顏面曲線發想，最適當的齒列位置，是不是前牙露得太多，會失去質感。過於外凸的嘴型，讓人覺得不夠斯文。必須讓齒列適度後移，讓鼻唇的曲線更顯立體，經過我們拔除小臼齒，將前牙適度後移，並以大臼齒的移動引領下顎咬合平面的旋轉，讓下巴的位置更為前方，讓外觀上更為立體與美感。

    我們不斷追逐矯正能帶來的顏面改變，就像鼻子與前牙之間的距離，因為適當的後移，讓鼻字更為明顯，透過矯正讓臉型產生極大的影響，而這樣的移動絕非ㄧ般牙科治療所能比擬。感受齒列移動造成顏面美觀的改變，是我們追逐完美的泉源，因為這能將平凡的東西化為神奇。

    本案例是ㄧ為非常害羞的病人在經過矯正之後，發現自己的前牙變得太直而且露牙齦，並且在臉部放鬆的時候有下巴後縮的感覺。

     就診斷面來說，由於矯正後上顎前牙的位置太過往內往下，前牙的角度太過內傾，矯正醫師稱為 loss torque control，造成牙齒的排列並沒有完全順著嘴唇的形狀。

    我們矯正的解決方法除了將上顎前牙反向轉回之外，更需要將整個上顎齒列往上後移，並且同時將下顎齒列向前以逆時針的旋轉下巴。同時因為下顎的旋轉，產生了肌肉的復位，而間接解決了下顎軟組織皺摺的“雙下巴問題”。

    下顏面的輪廓，因為牙齒的移動，讓整個臉型更為勻稱，猶如創作藝術般，沒有欲蓋彌彰的低俗感，簡約的線條，讓原本就突兀的外觀更為高雅。微笑時的產生的表情，讓患者更為自信與滿意。

    女兒的年紀還小，已經拔過小臼齒，其他醫生都說這只剩下這個手術才能達到效果了。我們除了藉由讓下排牙齒再度後移，試圖騰出上下牙齒之間的空間，再將下顎骨逆時針旋轉創造更明顯的下巴線條。我們藉由拔除下排牙齒讓下排前牙後移，才有機會讓上排前牙繼續後移。但上排牙齒仍需要藉由全齒列移動，才能解決拔牙空間喪失的問題。

    下顎骨的逆時針旋轉，下顎前牙的下壓，咬合平面的打直，更因下唇的適度後移，在極度有限的條件下，創造出較為明顯的下巴與微笑曲線。矯正過程中，也因為女孩年齡的增長，髮型的改變，更顯美麗動人。
