    ㄧ張簡單的側臉線條，訴說了追求美麗的決心。當我完成這樣的矯正案例時，想跟大家分享的是非手術解決極度外凸的案例。

   看似非開刀不可的齒列，藉由上顎前牙的大範圍移動，牙根及其周圍結構也因此像後上方移動，這樣的改變除了讓人中的角度更為自然外，下顎也因此可以向上方旋轉。下巴的線條秉持著創作畫作般的要求的優雅線條，因為齒列最後的適當位置，增添驚艷迷人的臉部自然線條。

    女兒的年紀還小，已經拔過小臼齒，其他醫生都說這只剩下這個手術才能達到效果了。我們除了藉由讓下排牙齒再度後移，試圖騰出上下牙齒之間的空間，再將下顎骨逆時針旋轉創造更明顯的下巴線條。我們藉由拔除下排牙齒讓下排前牙後移，才有機會讓上排前牙繼續後移。但上排牙齒仍需要藉由全齒列移動，才能解決拔牙空間喪失的問題。

    下顎骨的逆時針旋轉，下顎前牙的下壓，咬合平面的打直，更因下唇的適度後移，在極度有限的條件下，創造出較為明顯的下巴與微笑曲線。矯正過程中，也因為女孩年齡的增長，髮型的改變，更顯美麗動人。

『前牙牙冠的適度改正，產生了牙根往內移動，牙齒的適度後移，彰顯了原本美麗的表情。藉由智齒空間的大範圍移動，雙唇改變了鼻與下巴的距離，讓臉部表情更顯氣質。』

"Anterior crown proper correction makes the roots more lingual, which creates more natural look of the face. The space of wisdom teeth is closed by the distal movement of all dentition, which make the face with deposition."

    其實矯正治療中，並沒有簡單的案例，因為每位矯正案例都有其特殊性，都要注意牽一髮動全身的道理，所以我對所有的病患在諮詢前，都一定告訴他們，我們必定要經過充分的討論，用數據和科學來訴說完美的矯正解決方案，我們無法只憑經驗和想像來知道矯正時間、矯正費用和矯正結果。

 

露不出上排牙齒｜EXPOSURE PROBLEM 矯正露牙不足

    牙齒露不出來就無法展現燦爛的微笑與自信。造成露不出牙齒的原因可以是上顎前牙過高，下顎骨過長，咬合平面過於上彎，矯正失誤、整形手術等很多原因。

    而以下案例由cephalometric x ray 可以看出其上顎前牙非常的高，以致嘴唇開口較靠近下顎前牙。所以說話時微笑的動態情況，幾乎是看不到上排牙齒的。

    If the tooth exposure is not enough, we won't be able express confidential smile. And this is due to the wrong position of upper incisor or the length of lower jaw, occlusal plane problem, mistake of orthodontic treatment or failure of plastic surgery.

    From the following cephalometric x ray, you can easily notice the upper teeth is too high so that the opening of mouth is close to the lower teeth instead. Therefore, when the patient talk, the upper teeth almost can't be seen.

    We make the clockwise ration of the bottom teeth to create more space, so the upper teeth can go down. Making the smile and the face more than appropriate.

    露牙不足的牙齒排列是非常彎曲與傾斜的。由側方看去，牙脊的排列矯正醫師稱其為 curve of spee。 牙脊的排列在矯正後幾乎成ㄧ直線，優雅的曲線除了好看以外，咬合的功能，尤其是小臼齒與前牙區的接觸，可以有ㄧ個更適當更平衡的位置。

    The tooth alignment of less tooth exposure is usually very curved and tilted. From the lateral view, we can notice the alignment of teeth will become more straight. In orthodontics, we call this curve of Speed. Actually, when the cure become more straight, instead of tilting, the contact of upper and lower jaw will be better. This will create more balanced and beautiful bite position.

     本案例則為幾乎為牙醫師都會希望您可以搭配手術的案例，主因是牙齒位置過於前方及外凸外，露牙齦的程度為骨頭太長，上顎骨發育過度，幾乎不可能由單純由齒顎矯正治療改善！

     少數牙醫師會建議利用隱形牙套配合正顎手術，或搭配骨釘，但隱形牙套在先天的設計上，僅包覆到牙冠，較難移動到較長比例的根部，不如固定式矯正器能靠矯正線產生力矩，而能大範圍移動牙齒。隱形牙套雖然較美觀，但比較適合簡單到中等程度的牙齒凌亂或是有牙縫的不拔牙案例。至於像這類需要拔牙的案例、或深咬合、牙冠短、多處缺牙、短臉、嚴重小下巴、骨骼偏斜、#骨性暴牙、牙齦外露等案例不適合外。矯正時間可能加長、或必須搭配固定矯正裝置，而讓治療失去信心。

     我們對於外凸的臉型，葉醫師利用拔除第一小臼齒的空間來後移上顎前牙外，亦可同時將上顎大臼齒後牙區域上推，和下顎大臼齒下壓，以改變咬合平面，讓下顎骨向前方旋轉，以讓下巴看起來更突出。這雖然是很多醫師都已經知道的理論，但能真正駕馭成熟的醫師，應該是少之又少了。

     

    矯正治療是緩慢的，其中不知經歷了多少次因為質感追求與病人的配合，不因難以企求而放棄。但當我們做到了的時候，空間裡似乎產生了能量，自然而然與美融合在一起。生活就是不斷在追求生命中美好的體驗！而我們的矯正治療就是幫助人們對美的追求，而我們也希望每個矯正病患，都能感受到因為矯正的改變，讓其充滿自信與美麗。

『 矯正過程中產生牙齦外露？是不是越矯正越糟糕？』『矯正是否能解決露牙齦？』

牙齦外露伴隨在許多的矯正病人群中，而常常來診詢問治療的可能性。一般病患認為只能透過『正顎手術』或『骨釘拔牙』或醫美的方式解決這樣惱人的美觀問題。

但是我想『醫師本身只有認真地瞭解每位病人，用心聽懂她們真正想要的治療效果，而採用完整的治療計畫應該會離患者本身理想的治療結果相差不遠』。

本案例無正顎手術，無拔除小臼齒。全齒列移動解決。
