這是一位極難的雙顎前凸、開咬、骨性暴牙、咬合平面太傾斜，幾乎無法單靠矯正的案例。在後牙區，有了金屬與陶瓷假牙，但是因為『咬合』，讓牙齒都做的非常短小，幾乎不可能放上矯正裝置（尤其是右下方的後牙區）。這當然會是大部分矯正醫師完全無法發揮的案例！
